  • Has your pet received their annual vaccinations within the last year?

If not, we will provide basic vaccinations for free. Cats will need FVRCP and Rabies. Dogs will need Rabies, Distemper, and Bordetella

  • Please indicate vaccination status*
  • Are your pets spayed/neutered?

If your pet(s) is not spayed/neutered (surgically sterilized), this surgery will be provided free of charge. IMPORTANT: In light of the overwhelming number of homeless animals euthanized in shelters each year, we require your pet to be spayed/neutered in order to receive foster care or other services. If your pet(s) is older than 8 years, our vet will determine if surgery is safe. It is in the best interest of your pet(s) to get spayed/neutered, as it will eliminate their risk for reproductive cancers and can extend their lifespan. Female pets can pass away during or after labor. Additionally, unaltered animals often display unwanted hormonal behaviors which include "marking" indoors, bleeding, humping, aggression, and roaming to find a mate, increasing their risk of getting injured or killed.

  • Please indicate spay/neuter info*
  • Please keep in mind that spay/neuter is a program requirement. If you do not consent your application will be DENIED. Please go to our Pet Care Resource Center for options besides PICS if you cannot consent to this.
